How to request Scripps medical records
Scripps Health takes great care to ensure your health information is kept private and secure. With your authorization, there are several convenient ways to request that your medical record be sent directly to you or your physician.
For new patients
To transfer your outside medical records to Scripps as a new patient, please review the options below. It’s important to complete this process before your first appointment.
Link your MyChart accounts
If you’ve been seen at another health care organization, you might be able to view information from that medical record right in MyScripps.
After accessing MyScripps, look for “Link My Account” and follow the steps.
Once the accounts are linked, your Scripps provider will be able to see information from the other health care organization, including test results, medications, health issues, allergies and notes from your care team.

Upload records into MyScripps
If you have already requested and received access to MyScripps, and you have a digital copy of your other medical records, you can upload your records yourself into MyScripps.
- Login to MyScripps
- Click the “Menu”
- Scroll down to “Sharing” and click “Lucy”
- Click “Link me up with Lucy and MyChartCentral”
- To add a medical document from your computer, click “Add a Document”
- Click “Choose Files” and select the document and click “Open”
- Click “Next”
- Click “Save” and then “Next”
- Click “Save These Documents”
Once the documents are saved in Lucy, you can then share the documents with your provider.

If your other health care organization is not available to link in MyChart or you don’t want to link your accounts, you can mail your records to Scripps.
Submit outside records and images to Scripps Health
Medical records from external organizations not affiliated with Scripps Health may be submitted. Patients are required to fill out a signed authorization form from the external provider and have your external provider submit it through the following to Scripps via mail, email or fax:

Images from external organizations not affiliated with Scripps Health may be submitted via mail or in-person to the Scripps Green Radiology department.Please note that any images submitted to Scripps will not be returned unless specific arrangements are made.

For Scripps patients
To request your Scripps medical record to be sent directly to you, your non-Scripps physician or other authorized person, please review the options below.
View, download or request records and images through MyScripps
View and download:
If you’re a Scripps patient and use the MyScripps portal, you can view or download visit records for personal use or send visit summaries to a provider.Log in to MyScripps and look for the “Document Center” section under the menu.
Images can be viewed by selecting “Test Results” under the menu.Scripps Health currently provides access to X-ray images for patient viewing purposes only.
Request records:
If you’re a Scripps patient and use the MyScripps portal, you can request copies of your records and imaging through MyScripps.Log in to MyScripps and look for the “Request Medical Record” section under the menu and complete the steps within the “Sharing Hub.”
Records requested will be delivered to your MyScripps account and may incur a processing fee.Imaging is shared through Nucleus.io, a web-based, secure cloud platform.Imaging requests will not be processed without a valid mailing or email address on the MyScripps request form.
Request records and images through the Scripps Release of Information Department or in-person
If you don’t have a MyScripps account, you can submit a paper request for your medical records or radiology images by following the steps below.Please review both pages of the form thoroughly prior to signing, and be aware that whoever you give your health information to must be responsible for securing it.Please note that Scripps Health maintains records for 10 years and 25 years for moms and babies.
You may ask that the authorization form be mailed to you by contacting the Release of Information Department at 760-633-7746.

Emails requesting medical records must include a completed Request/ Authorization for Health Information form as a PDF attachment (download the form above).
Completed patient requests for Health Information may be dropped off at any Scripps Radiology department location, Monday-Friday, 8 am - 3:30 pm.Requested information will be sent to the recipient named on the completed authorization form. Same-day pick up is not available.
Patient costs for records
There is no cost for medical records if they are sent to another health provider for continuing care.
If medical records are requested and released directly to the patient, or an authorized representative, for personal, legal or insurance purposes, Scripps Health charges the following fees:
- $5 for each patient request
- $0.02 per page charge for electronic records over 250 pages
- $0.10 per page for non-electronic records over 50 pages
- $6.50 for radiology images CD copy or Nucleus.io Imaging Cloud upload

Important information about requesting medical records
Authorizations signed by a patient representative must be verified by including a copy of one of the following documents:
- Legal guardianship papers including, but not limited to, court order(s) or conservatorships.
- Designation of Personal Representative form completed and signed by the patient.

- Advanced Directive for Health Care specifying agents’ authority to request medical records.Fill out the form (English (PDF, 160 KB)/Spanish (PDF, 180 KB)) and give copies to your designated representative, physician and health care institution.
- Requests for medical records of deceased patients require a copy of documentation verifying personal representative status.Requests for medical records will require a copy of the death certificate.The person identified as the informant on the death certificate (designated as line item 26)has the authority to sign the authorization for releasing records on behalf of the deceased patient in cases where legal documentation is not available before the time of death.
Serving subpoenas to Scripps Health
All subpoenas for medical records must be hand-served in person with a $15 payment at the Scripps Health Release of Information Department:

There is a $15 processing fee for health record requests from legal offices, copy services, and insurance companies (additional fees may apply). Payments are to be made out to Scripps Health: Release of Information Department. To pay by credit card, please contact the Release of Information department at 760-633-7746, Monday through Friday 8 am - 4:30 pm.
Hospital records:
Subpoenas must be served separately for medical records and images and must be made out to the individual hospital, including name and hospital address.
Clinic records:
Subpoenas must be served separately for medical records and images and must be made out to Scripps Clinic 10666 N Torrey Pines Rd La Jolla, CA 92037 to encompass all Scripps Clinic and Scripps Coastal Medical Center locations at Scripps Health.
